    6. Form for Confirmation of Registration in the Ph.D. Programme

  • This is to be submitted only once.

  • Every registered Ph.D. student has to submit this form.

  • This form is to be submitted immediately after completion of course work (usually 1 / 2 semesters) and/or after Ph.D. Qualifiers (as applicable). The copies of the semester grade cards should be attached alongwith this form.

  • Attested copy of Qualifying Degree Certificate has to be submitted along with this Form (if not submitted at the time of Verification of Documents)

  • Usually, the students with the qualifying degree as ME/MTech or equivalent has the course work of 1 semester. Therefore the students with this background should submit their form after successful completion of 1 semester course work.

  • The students with the qualifying degree as BE/BTech/M.Sc. or equivalent has the course work of 2 semesters. Therefor the students with this background should submit their form after successful completion of 2 semesters course work.

    7. Course Registration Form for External Ph.D. Students

  • This Form is to be submitted every semester (if not registered online).

  • This form is for External category students who joins back their Parent Oraganisation after completion of Course Work.

  • Every student has to register Online/Offline for each semester till submission of Synopsis & Thesis.

  • If External category students are not on the campus during the registration dates then these students should download this form and send the duly filled form for further processing.

    8. Ph.D. Annual Progress Seminar (APS) Report Format- Cum - Extension of Ph.D. Registration Form

  • This report has to be submitted Annually (in August or January as the case may be) till presentation of Pre-Synopsis Seminar.

  • Every registered Ph.D. Student has to give APS till presentation of Pre-synopsis.

  • This is an evaluation Report of RPC on the work presented by the student after completion of each academic year.

  • The students joined in Autumn Semester (July), after completion of 1 year have to present their APS latest by 31st August, of every year, till submission /presentation of Pre-synopsis. The students joined in Spring Semester (December/January), after completion of one year, have to present their APS latest by 31st January, every year, till submission /presentation of Pre-synopsis.

  • The APS report should reach the Academic office within 4 days from the date of presentation of APS.

  • Fine will be levied for late presentation of APS. A student may apply/request for extension of APS to PGAPEC 15 days before the due date of APS presentation with appropriate reasons, through his/her supervisor & Head, of the Department.

    9. Ph.D. Presynopsis Seminar Examination Report Format

  • This is to be submitted only once.
    (However, in exceptional cases, where the pre-synopsis presentation is unsatisfactory OR on the recommandation's of RPC/PGAPEC this may be submitted again).

  • This is a Pre-synopsis report. The student who has presented Pre-Synopsis seminar has to submit this report format to his Supervisor/RPC at the time of seminar. The RPC will forward this report to the Academic office for approval of PGAPEC.

    10. Forms for Submission of Ph.D. Synopsis and Thesis

  • This is to be submitted only once.

  • This form is to submitted at the time of submission of Synopsis & Thesis, alongwith the copies of Synopsis & Thesis. In addition, the pdf file of the synopsis has to be sent by email to the Assistant Registrar (Academic) [email : aracad@iitb.ac.in] with copy to concerned dealing assistant in academic office [email addresses of the academic office staffs are given in academic homepage. www.iitb.ac.in/newacadhome/offdetail.jsp

  • The form includes Synopsis submission form, Thesis submission form & Panel of Examiners. The student must attach following two clearances certificates along with this from :
    1. Hostel (Mess Clearances)/HCU and
    2. Accounts Clearance certificate
  • The student must also submit attested copy of Qualifying Degree Certificate along with this form.

    11. Form for Final Submission of Ph.D. Thesis (After Defence/Viva-voce Examination)

  • This is to be submitted only once.

  • This form is to be submitted at the time of final submission of Thesis. After the defence, the student is required to submit the TWO hard bound copies of the final thesis.

  • The form includes No Dues Clearances (NDC) certificates from the Department,Library, & Hostel/Estate office.

  • The student is required to complete the required formalities / pay dues, if any and submit the NDCs from all necessary Departments/Section to Academic Office to become elligible to be included in the Convocation list.

  • Student must note that after successful completion of the Ph.D. Viva-voce examination, the student should submit to the Academic Office the two copies of his/her approved thesis, duly bound, together with this application within TWO months , from the date of defence, failing which a fine of Rs. 5000/-per month will be levied to the concerned student.

    12. Convocation Form

  • This is to be submitted only once.

  • This is to be submitted online.

  • Every student, who has successfully completed the requirement of Ph.D. Programme has to submit this form.

  • After successful defence, the student has to submit this form online. The printout of the filled ONLINE form dually signed is to be submitted to the academic office for records.

  • This form can be submitted offline (downloadable form), with attachment of a copy of the receipt of the convocation fees paid in the cash section.

  • The Convocation of the Institute is held annually in Ist/2nd week of August every year.

    16. Request for Change of Category

  • The student may require to change his/her existing admission category to new category for various reasons. The request should be made to PGAPEC in this format.

  • Students must attach appropriate documents (For eg. In the case of change of category to External (EX), student is required to submit External Supervisor's Biodata, External Supervisor's consent, and NOC from the Employer; For Change in category to TA/CSIR/UGC, a copy of GATE/CSIR/UGC score card/award letter etc.) for consideration of PGAPEC for approval.

  • This format is to used as and when required.

  • The consent of Existing (old) Supervisor(s) and Proposed (New) Supervisor(s) is necessary.

  • The format can be used for inclusion of External Supervisor for the Ph.D. work. This student is required to submit External Supervisor's Biodata, External Supervisor's consent, and NOC from the Employer.

    18. Request for NOC for VISA for attending International Conference/Symposium outside India

  • This format is to be used as and when required.

  • The Approval of Dean (AP)/Asso. Dean(AP) be sought before submitting this form in Academic Office.

  • Student is required to attach copy of Proof of Invitation/Acceptance by the Organiser of the Conference/Symposium for attending/presenting his/her paper/abstract.

    19. Request for Extension of Annual Progress Seminar (APS)

  • This format should be submitted 15 days before the due date of APS presentation.

  • This is an application /request for seeking Extension for presentation of APS after due date.

  • If the request has to be made on the Bonafide reasons/Medical grounds, the relevant documents/certificate are to be attached as a proof along with this form.

  • Students must note that if the APS is presented after the due date, a fine of Rs. 750/- per week is applicable.

    21. Application for Temporary Withdrawl from the Ph.D. Programme

  • The Student, who need to take temporaray withdrawal from the Programme has to submit this form.

  • Temporary withdrawal from the PhD Programme, for a maximum period of one year or two semesters, is admissible after successful completion of at least two years in the Programme from the date of confirmation of Ph.D. Registration. The process of confirmation of PhD Registration should normally have been completed when seeking withdrawal. The student must state the period during which temporary withdrawal is desired, the grounds on which temporary withdrawal is being requested (along with the documents/certificate as proof) and obtain recommendations from the PhD Supervisor(s) and the Convener, PGC before submission of this form to the Academic Section.

    22. Request for Partial Financial Support from IRCC for Completion of Synopsis & Thesis (After successful presentation of Pre-synopsis Seminar

  • This is to be submitted only once.

  • This Form is to be submitted after successful presentation of pre-synopsis seminar and completion of tenure of TAship/RAship/Fellowship.

  • The Financial support is given from the date of presentation of pre-synopsis seminar, for a maximum of 3 months or till the date of submission of synopsis and thesis, whichever is earlier, on the basis of attendance certified by the respective academic unit.

    23. Request for Extension for Submission of Synopsis & Thesis (After presentation of Pre-synopsis)

  • After presentation of Pre-synopsis, a student is expected to submit copies of Synopsis/Thesis within Four months from the date of Pre-synopsis presentation.

  • However, if the student is not able to submit Synopsis/Thesis within this period of time, he/she should request for extension in this format.

  • OR
  • If the student is not able to submit Synopsis/Thesis by the due date of submission of Synopsis/Thesis, as granted/permitted by the PGAPEC, he/she should apply for extension in this format.

  • The student should seek prior approval of the competent authority before joining any University/Organization/Industry.
